Exercise
After seeing the new doctor last week, I felt like a freeze had been melted on my exercise (or lack of it). I think it just put me in the right frame of mind to get back to what I needed to improve my health. Anyway, I have started back to riding my bicycle again since that’s what I most enjoy for exercise. In recent months the only other significant exercise had been swimming some of the time I took my client (as a carer) to the pool.
I had to prepare a bit before riding as my bike’s been sitting unused on our patio for a long time. The tires were flat and it was very dusty. It’s also been rusting more and more after being outdoors for so long. I want to get a new bike some time and give this one away. I’m sure someone more able to could fix it up just fine and it would be great for someone.
A local highway has been extended near us and some parts have already opened. There’s a lovely bike/pedestrian path that runs alongside it and it goes halfway to another suburb that is kind of out in the country. But not really in the country any more. Just not quite city either. Anyway, I wanted to try out the new path and it seemed I could get there without too much trouble from home. So I tried it out and it’s lovely. Mostly flat and straight so there was little strain on my knee. That’s been what has put me off riding a lot because I just hadn’t had strength for riding up and down hills like I had been doing. I was able to keep a steady pace without getting too sore. At one point the path loops around and goes under the highway. This part runs alongside a road which I can’t identify yet. I think it might be one that goes to a local Aboriginal community but there’s no signage in that area to be sure. Anyway, the path continues on the other side and then it ends abruptly near the remaining construction on the road. It will be good when it’s finished because there will be an easy way to get to the next regular suburb in that direction.
On the way back I realised there’s an overpass for cyclists and pedestrians being created. It’s mostly done but the paving isn’t completed. Also from this direction there will soon be open good paths to reach Whiteman Park near us. I have gone there by bike in the past but it was scary to ride alongside the sometimes heavy traffic. Now there’s a pathway going straight to the park and into the park. Much safer now. The highway is supposed to be opening the section closer to us this weekend so I hope to make a journey to Whiteman Park soon, unless the pathway is still blocked.
I’ve done two bike rides and have done swimming this week, which is far better exercise than I’ve done for a while. It’s one of those things where it’s easier to continue once I get going on a regular basis. I’m hoping that I can keep it up. I’ll be seeing my new gp for follow up on Monday and hopefully that will help keep the momentum going.

Changes
When we first moved to Australia in 1994, I was halfway through my first pregnancy and started seeing a gp in the practice the Scientist’s family had been attending. I must admit it was a very odd experience seeing a doctor here. It was all much more relaxed and patients even would call the doctors by their first names sometimes. There were none of those paper gowns I’d grown up experiencing. When a gown was needed, it was cloth. And for the most part, there were none of those exceedingly long waits for the doctor to finally grace us with his or her presence. Initially we went to the same practice the Scientist’s family had been using. I went to the female gp, who was the wife of the one the Scientist had seen for years.
Eventually we switched to a practice closer to home and we were happy enough there for years. It went from a small practice with one or two doctors to a medium one a few years later with six or seven. We were still pretty satisfied there even though it was a lot busier. I’d had the mistake of going to the registrars a lot back then. I had several really good doctors but then they would shift to other practices or finish their training and I’d have to find someone else there. I did finally have a gp I saw regularly for ten or more years. I liked her a lot although I would never say I loved her. At some point I needed to see a doctor more urgently and found myself with a different gp and I really loved her. On top of that it was easier to get appointments with her and I could use online appointment apps to get them. It was all going quite well and then that doctor disappeared and I was back to square one. By this time the practice had moved into a purpose built structure and had become really large and sterile. It was all very modern but truly a sterile place. I moved around from one doctor to another the past couple of years but haven’t found anyone I was really comfortable with. My previous gp had gone on maternity leave and come back to a lower patient load. I had discovered by accident that the doctor I’d really liked had started at another practice a bit further away. I had almost made an appointment there and done the extra travel but then I found she wasn’t taking new patients. And then more recently I found she was no longer at that practice but now at another one and I pondered what reasons for another change could be. I decided it was for the best I hadn’t tried to follow her in the end since this could be a regular problem. Anyway…
The Scientist and I talked from time to time about changing to a new practice for various reasons. The problem is finding one we would like. Also, it’s hard to leave a place we’d been using for over twenty years. The Scientist had been really ready to jump ship well before me but last year some major medical issues came up and that was put on hold. Meanwhile I had been trying out different doctors to see if I could find a good fit. It just wasn’t happening with anyone I was seeing.
In the past year or two, my diabetes management hasn’t been all that great. I ended up on medication for the first time when diet and exercise weren’t enough. I won’t say I was doing everything perfectly but even at best I still was struggling to keep my blood sugar in range. I had seen one gp who was supposed to be good with weight issues and diabetes. In some ways I liked her but I wasn’t totally comfortable. The last time I saw her she told me we’d have to look at insulin should things not improve after a few months.
So what happened is I was up and down on doing things right and I procrastinated a lot. And then recently I decided to seriously look at other doctors and see where I could go. Finding sources of ratings wasn’t easy but I did find a few and looked at one place that’s a bit of a drive (compared to a local one) and then stumbled on a practice that’s in the next suburb. Turns out all the doctors seem to be female, whether by design or not, I’m not sure. One of them listed herself as having a wife and there’s another doctor there with the same surname so I suspect that is the wife. It’s always nerve-wracking trying to decide on which doctor to try just by reading what’s listed online. In words the doctor sounds great but you really can’t be sure if you will find a good connection. Anyway, I need a new prescription for my diabetes meds so made a choice and went in yesterday.
At first I was a bit put off when the receptionist made a point of reminding me it was a private billing practice so there would be out-of-pocket expenses. It wasn’t that this was the case but the way it was said that bothered me. I was fine with it and said so then filled out the paperwork and waited. I didn’t get called back right on time but it wasn’t long after. The doctor I’d chosen is originally from Germany but has been in Australia over twenty years. I was expecting a European accent but instead she sounded more like someone from the UK, maybe Scottish or Irish…weird.
Anyway, she started off by apologising that she’d need to go out at some point as there was some sort of urgent situation she needed to attend. But she said she didn’t want to keep me waiting so had brought me in to get started. She was really good and went over my medical history and covered heaps of information. In the middle she did have to go out but she again apologised when she returned and again at the end.
With regard to my diabetes I did admit to procrastinating about following up on blood work from before. I mentioned I’d been put under threat of insulin but had been up and down in my efforts at improving my diet and exercise. Anyway, she told me that it wasn’t about whether I was doing well or not, I needed to follow up and get treatment. She also pointed out that there was lots more we could try before insulin. I do feel the previous doctor was using scare tactics…but I guess that didn’t work. She asked if my meds were time release or not and I didn’t know but assumed not since I was taking it multiple times a day. She went on to say that most patients do better with the time release. I wondered why this was never mentioned before. The other thing about time release is that it’s easier on the stomach which also means it possibly would help decrease irritable bowel issues. I again wondered why this wasn’t mentioned before. She also made a point of showing me how the fact I don’t smoke makes a huge different on my cardiovascular risk and how I shouldn’t focus on what I’m doing wrong all the time. She made a lot of good points which made me feel better about my efforts or lack of efforts…which just made me feel better in general.
I left that appointment feeling like I surely made the right decision to start anew. And I kind of wish I’d done this years ago.

Cough, cough, cough
Game Fanatic is still coughing badly. I took him to see a doctor for the third time this morning. The appointment started out as mine because my sore throat didn’t seem to be improving after over a week or so. It now seems to be improving and swallowing isn’t as uncomfortable as it was. I didn’t know this for sure when I changed it online last night but I must be getting better. I still have my own cough happening but it’s early days and post-viral coughs do sometimes go on for ages. This is especially true this year.
I didn’t think a 10am appointment would be an issue for Game Fanatic but I had to almost drag him from bed to the doctor. He did get up but it was slow going for quite a while. The doctor is the same one I’ve seen the last few times and she was pretty thorough at checking him out. One of his ears is pretty red and blocked and she couldn’t see the other eardrum due to wax. It’s hard to tell about his lungs because he didn’t breathe in deeply enough for her to hear properly. I think he was worried it would start off a coughing fit. He probably wasn’t wrong. I suspect his lungs were okay since that was the case the first time he was in to see a doctor. So we have a script for another antibiotic but she thinks (and I agree) that it’s most likely viral and will be useless. But it’s there should he get worse and need it. I assume that’s more about the ear than the cough since the cough is already pretty awful. Also, he’s supposed to take Vitamin D supplements as his was low when tested earlier in the year. I don’t recall mention of this before but maybe I have forgotten…that does seem to happen to me sometimes these days. Sigh. Ideally more sun would be most beneficial.
I happened to mention my cough which started from a sore throat and she said that the coughs she’s been seeing haven’t been connected with sore throats this year. I guess I get to be different. :D
Lego Lover seems to have missed out on any cough or cold this year. At least that’s the case so far. He seems to have gotten the breathing difficulties out of the way earlier in life and hasn’t had any asthma issues in years. In fact, that boy has been pretty healthy for several years despite his poor diet and exercise.
Funny that the first of the month is considered the first day of spring here and it’s been pretty chilly. Tuesday night the temperature dipped almost to freezing in the night and the day time temperatures didn’t get that high either. This is all after a somewhat mild winder overall. We’ve had quite a bit of rain lately so I imagine the spring allergies are going to be loads of fun this season.

Happy 2014!
I have been missing in action for the past several weeks. I’m here but for some reason I’m at a loss of what to write quite a lot of the time.
We’re just getting through the last month of summer here and it’s been fairly mild for the last couple of weeks. Not to say we haven’t had some hot days but nothing extreme like has occurred in many of the eastern parts of the country and none of the nasty floods that have hit the north of our state. I wouldn’t mind, however, having a small share of the rain from the north. After such a wet late winter, it’s been almost completely dry here since the end of October, I think, perhaps a bit earlier.
January wasn’t a great month for us. The year started off well enough but there were a couple of things happening, family-wise, that left January a rather negative month. I can just hope the year improves from this point.
At the end of the month, our gym membership contracts will run out and we are cancelling our membership with the club. Not that we are unhappy there, but the Scientist reached his goal months ago and I have started to lose much interest in going. My aim at the beginning was to go at least twice a week and aside from a couple weeks at Christmas, I have more than stuck to that for the better part of the eighteen months. Now I am more interested in doing exercise outside the gym like just walking or dog walks, cycling, etc. We also now have a spin cycle and a treadmill in the house, so should be set for even the winter and the really hot days left in the season.
Speaking of the gym, I am still working on my weight and fitness levels. I think the fitness is still pretty good but the weight stalled for quite a while and I have had to overcome some of the psychological aspects of that before getting going again. I also had some shoulder bursitis and tendinitis issues that I couldn’t quite get sorted with physiotherapy alone and had a steroid injection just before Christmas. With further physio, some of the shoulder issues have improved enough so I just need to make sure I don’t overdo any particular type of exercise again. Anyway, after some ups and downs, I think I am back on track for the moment.
I saw my new immunologist for the second time on Saturday. She had requested I have certain blood tests done and the results were back well before the appointment. They also sent copies directly to me. Overall, the results were all fine but I can’t help but be a bit pedantic about one thing that showed up. I’ve been getting a high titer ANA back every test since the first one back in 2004. This time was no different but there was a completely different sort of pattern to the ones that have appeared before. It was a different lab so my feeling is it was misread. My doctor wasn’t bothered about, didn’t really seem to know the pattern differences either. My usual pattern is rare but associated with Sjögren’s syndrome (which I have) and also autoimmune liver disease. Because of the pattern, previous doctors were keeping watch for a couple of liver-related antigens in blood work. These never were positive in the past but one associated with autoimmune hepatitis did come back a weak positive this time, which is rather funny, I think. At the moment, it isn’t so much of an issue due to it being weak-positive but mostly due to the fact almost all my liver functions tests came back in normal range. Only my albumin was slightly low…so also isn’t an issue at this point. At the moment, I will just go back for yearly reviews as I’ve done since my symptoms haven’t been anything major as yet.
Today I went in for a whole blood donation and was asked to give plasma instead. I was a bit surprised since it’s been so long since they’ve requested whole blood. Plus I thought they really wanted it this time as I got more reminders than usual. I’ve become acquainted with some of the staff at the center since I began donating, to the point that some of the staff skip quite a few of the questions. But the last couple of times, I have gotten new people and they have gotten into detail on some questions. Strangely it came out today that nobody thought to note down the Sjögren’s syndrome on my records before. The lady that interviewed me had never even heard of it so then went on to check it was okay to donate. Which it was.
Our board games have sat rather untouched the past two months. At first, Lego Lover wasn’t interested in playing but then we missed it one weekend or another and just haven’t got back to it. Meanwhile, we’ve had a few more games arrive lately so we need to get back into it.
There are probably some other things I need to update but honestly can’t think of anything now. I just wanted to get back into it keeping track of things a bit more before more time passed.

Different sort of challenge
I signed up a couple days ago to be part of the Go Red for Women heart challenge sponsored by the Australian Heart Association. I don’t seem to be able to maintain the better eating and exercise habits so I’m hoping this will help keep me motivated for a bit longer. It starts on the first of next month but I’m not necessarily waiting to start working a bit harder on my goals. I’m mostly looking just to improve my fitness and my eating habits since experience has shown the body shape and size will adjust quite easily with those changes in place. It’s possibly a bit risky with the seasonal change towards wet weather ahead but I’m planning on using walking as my main form of exercise. And just maybe I will get my bike into a shop and get it serviced so I can feel safe while riding again.

Dragging along
It’s been rather a chilly start to spring around here and I’ve been lacking energy lately. I’m not getting much exercise and I’m struggling to keep from going overboard on carbohydrates (my big weakness, food-wise). I can’t help feeling that if it warms up and turns into spring proper, I might come back alive.
Meanwhile, I’ve been battling a blocked left ear for a couple weeks now. I had a reprieve for a few days but it’s back now. I’m also having regular stomach pains and haven’t been able to figure out the reason. The Scientist keeps saying I need to see the doctor but I figure it will suddenly disappear about the time I do that. I think the ear problem is likely related to reflux…maybe the stomach problem is also related.
Slightly-related, I’ve been considering going dairy-free for a while to see if it helps with some of the troublesome symptoms I have. While most don’t keep me in bed, they are rather disruptive to everyday life. I’ve not done this as yet because I’ve not been able to find a satisfactory alternative to milk. My previous attempts to drink soy milk have not gone very well. I just haven’t liked the taste at all. But I did try some with chocolate flavoring added this weekend and it was actually pretty decent. So maybe I should revisit soy milk as an alternative.

Cold hands and feet
This morning I took a bus to an appointment at the teaching hospital near Perth. I had to be there at 9:45 so the Scientist dropped me off at the bus station where I caught a direct bus. Although the temperature wasn’t really all that low, it sure felt cold this morning. The bus trip was mostly uneventful and I arrived about twenty five minutes early. I went ahead and signed in at the clinic since there was little else to do. I’d barely sat down when I was called back to sit in the corridor outside the office. It was pretty quiet when I arrived but that was soon to change.
Some patients had been there long before I arrived and were still waiting. One man nearly walked out and another woman wasn’t able to be seen because something was missing from her file and they had to wait for the medical records department to send it through. She was very upset by the time she went in, just before me. Both had been there for appointments scheduled for 8:30 am. Having experienced a lengthy delay in the past (due to an admin. error), I came prepared for a long wait. I used my pda to continue reading Great Expectations. My only concern was that the battery would conk out early and I didn’t have a spare. Fortunately I was called in before this occurred.

Health and fitness
Since my last post with regard to health and fitness, it seemed not a lot happened for quite some time. My decision to make sure I changed my food intake and exercise just didn’t seem to get me very far. I wasn’t failing exactly, but I wasn’t progressing and it seemed my weight continued to increase, although very slowly. My exercise intentions finally started to stick a bit better in the last couple of weeks. I’ve been using my bicycle as often as possible instead of taking the car on short trips. Today I cycled off to the pharmacy to get something for Lego Lover’s throat and the trip there was only marginally longer than driving. So I saved fuel and wear and tear on the car…and I expended a few calories at the same time.
The bicycle is getting far more use lately so I hope to continue to that habit. I did have a bit of a setback on Friday when I rode up to the shops, locked my bike and returned to find it moved…Some teenager had picked the lock and moved it but left the lock and my helmet in the same place. He had a bit of an attitude and was openly bragging about his act. Too bad he didn’t put his talents to better use. So I will be driving off to find a better lock this week before use it for anything longer than a very quick stop.

GP review
I saw my gp this afternoon to review lab results and to discuss my ongoing cough and reflux issues. The good news is that my labs results are excellent. My glycated hemoglobin was 5.0% which is the best result I’ve had so far and was again reported as being consistent with a non-diabetic individual. Ironically, I have been eating far more sugar and probably a bit more carbs in general. My thyroid was good and my cholesterol had improved over my results from a year ago (before I altered diet and exercise). So, it was all good.

Return
As I write this, my mother-in-law is on her way home to Perth from overseas. Her flight is due in around 1:40am, about 25 minutes from now. Yes, I should be sleeping but I’ve had a run of really late nights and I am finding it hard to readjust to an earlier sleep time.
My father-in-law phoned from to say that she was on her way home and wanted me to make sure the scientist’s brother W was there to pick her up at the airport. He’s rather hard to reach at the best of times and today he had left his mobile phone at home. So I phoned him at work (he is at the same company as the scientist). This got rather confusing because the scientist had left work just before I received the phone call and I had to clarify to the boss who answered that it was his brother I needed rather than the scientist.
